Enrollment Application

  1. Complete this Adult Career Training Enrollment Application (Word File).
     
  2. Mail the form to:
    EMCC Counseling Office
    Attn: Counseling Secretary
    41 O'Connor Road
    Fairport, NY 14450

If your application is approved, you may be contacted to schedule a TABE test.
